We offer wide range of nursing services and are always expanding our services. Some examples of nursing care services that we offer are:

  • Skilled Nursing observations (your home, surroundings for safety and health concerns), assessments (your physical, social and health assessments) and evaluations (care plans based on assessments).

PERSONAL CARE SERVICES INCLUDES:

Our Personal care Aides offer variety of services, skills and responsibilities. Some examples are:

  • Assist with Activities of Daily Living (ADL’s)
    • Bathe/shower
    • Assist with feeding
    • Assist with dressing.
    • Skin care
    • Oral care
    • Peri care
    • Catheter care
    • Assist with transfer and ambulation
    • Assist with turn or reposition
    • Change bed linen, clean bedroom
    • Prepare meals and snacks
    • Assist with laundry

  • Other Home Health Aide services
    • Light house keeping
    • Assist with transportation and appointments
    • Overseeing Safety for the Client at Home
    • Collect and relay information about conditions and relay it to the family and caregivers.
    • Provide and empty bedpans, empty drainage bags, catheter care
    • Observe patients for skin condition, blood in urine or other injuries/wounds and report.

CARE MANAGEMENT SERVICES:

  • Assist patients with healthcare needs following discharge from Hospitals or Skilled nursing homes or Rehabilitation centers to home.
  • Discharge instructions follow through.
  • Care provided by interdisciplinary care team (Skilled nursing, therapies, Medical social worker, Home health aides) as appropriate.
  • Assess home safety
  • Assess and manage potential accommodation
  • Reconcile medications and communicate to primary care provider.
  • Identify and offer community resources
  • Ongoing patient and family education and training
  • Assist with physician appointments and resource for transportation
