Our Chronic care and maintenance program helps diabetic patients to monitor and manage diabetes at home preventing complications and hospitalizations. Our care team identifies and generates patient centered individualized care plan to manage diabetes. STROKE
Thousands of people suffer from cardiovascular stroke or stroke each year. Most of the survivors require rehabilitation and interventions to increase their functionality and quality of life. Our chronic care program helps patients who have suffered from stroke. Our care team of skilled nurse, physical therapy, occupational therapy and speech therapy creates an individualized care plan focused on quick recovery of the patient and meet their needs.
